Source text - English If I had to summarize Translators without Borders’ learning from the Cyclone Idai response, it would be: language support can be a significant tool for effective, accountable humanitarian action. But only if there is a more comprehensive approach to community engagement from the outset. It is one thing to read a statistic about the linguistic diversity and low literacy levels of the population in Mozambique. It is another thing entirely to sit down with a group of Cyclone Idai survivors in Beira and hear it in person. To learn from one person after another that they are unable to communicate with aid workers in a language they understand. This is what TWB’s assessment team and I heard a few weeks ago when we conducted a rapid language assessment in four temporary accommodation sites. We found that many people do not understand the main languages and formats used by humanitarian organizations. They voiced frustration about how difficult it is to access information about available assistance. After one of southern Africa’s worst disasters in decades, we learned that much humanitarian communication is failing because it is in the wrong language. Today, in the comfort of my home, I’m thinking about what this means. In a way, it shows that humanitarians still fall short of meeting their commitments to “leave no one behind” and “put people at the center.” This is probably not news to many. But it leaves me torn when thinking about the impact of TWB’s language support services in the Cyclone Idai response. Source text - English What is this? The COVID-19 pandemic has increased demand on hospital emergency departments. Research into alternative ways of managing less urgent cases (such as walk-in centers and General Practitioner cooperatives) may provide information that would help ease this demand. In this systematic review, the authors searched for research on the effects of walk-in centers and General Practitioner (GP) cooperatives on local ED attendances. They restricted their search to articles published in English between 2000 and 2014. They included 4 studies of walk-in centres (all done in the UK) and 7 studies of GP cooperatives (which were done in Australia (1 study), Ireland (1), The Netherlands (4) and Switzerland (1)). What works: GP cooperatives significantly reduced non-urgent ED attendances. What doesn’t work: Nothing noted. What’s uncertain: The effects of walk-in centers on ED attendance are uncertain. They included 1 randomized trial comparing a call-back telephone consultation with a same-day face-to-face appointment, one systematic review (including one randomized trial and one observational study) of telephone consultation and triage, and one systematic review (including one randomized trial and 4 observational studies) of telephone consultation. What works: Telephone GP consultations provide a suitable alternative to face-to-face GP consultations in some settings. Triage in telephone consultations may reduce workload in some GP settings. COVID-19 is placing great strain on healthcare workers but maintaining the quality of care and patient safety is crucial. Therefore, it is important to understand how patient-mediated interventions can influence healthcare practice. In this Cochrane systematic review, the authors searched for randomised trials of the effects of patient-mediated interventions on the performance of healthcare workers. They did not restrict their search by date or language of publication and did the search in August 2018. They included 15 individual randomised trials and 10 cluster randomized trials (total: 12,268 patients), and also identified 2 ongoing trials. What works: Patient-reported health information interventions are likely to improve the quality of healthcare services. Patient education interventions are likely to improve the quality of healthcare services. What doesn’t work: Patient decision aid interventions have little or no impact of healthcare workers’ adherence to recommended clinical practice. What’s uncertain: The effects of patient feedback about clinical practice, patients being members of committees or boards, or patient‐led training or education of healthcare workers are uncertain. Across all patient-mediated interventions, the effects on adverse events, desirable patient health outcomes, patient satisfaction and resource use are uncertain. Established plans to manage large increased patient numbers may result in more effective service provision in the event of a pandemic illness. In this systematic review, the authors searched for research that evaluated hospital surge capacity in emergencies and disasters, specifically in hospitals with a preparedness approach. They restricted their search to studies published in English language peer-reviewed journals from 2000 and did their search in November 2015. They included 1 randomised trial, 2 qualitative studies and 14 cross-sectional studies. What was found: Actions in the following key domains may increase hospital surge capacity: staff, stuff, structure and system. Ways to increase surge capacity include recruiting student and retired staff, reallocation of equipment from non-essential areas to key areas, and preparation of temporary treatment areas. Appropriate planning is necessary because increasing capacity without appropriate measures, such as training volunteer forces, may adversely affect overall service capacity. What’s uncertain: There is a lack of consensus on the definition, classification and evaluation of hospital surge capacity.
