According to the Duden, regelhaft = "sich nach bestimmten Regeln vollziehend". Unfortunately, it is not infrequently used in the sense of "regelmäßig" (= in festen Zeitabständen wiederkehrend), and this may well be the case in a physician's report, because we doctors tend to avoid 'normal', down-to-earth words ;-).
Still I think that the point here is "according to the rules" (of kyphoplasty), i.e., "proper restoration of the height of a collapsed vertebra is not possible" with the technique critisized.
(However, if there is a chance of finding an appropriately vague English term which covers anything from "not always" to "not properly", go for it!)

Does "in a reproducible manner" fit into this scheme of things IMO?
I don't know, it covers mostly the "regelmäßig" part I think. Is there any chance that "regularly" may have the same ambiguity between "always" and "according to the rules"? Otherwise I think I would offer the client two versions, "proper restauration...is not possible," and, "restauration is not usually possible," and ask him what he had meant to say.
NB: I like the "as per" construction in your second suggestion above. For me, it has the same slightly blown-up touch as the source text.

: Ja: mehr Information, aber im Kern ähnlich wie mein Vorschlag: sprachlich eigentlich "according to the rules"= normal, aber mit Unsicherheit, ob nicht doch einfach nur lax "usually" gemeint ist

Danke, Steffen - ja, halte ich auch für möglich (s.o.), aber mit den "Grundbedingungen der Kyphoplastie" zusammen für die weniger wahrscheinlichere Variante. Wir werden es ja vermutlich von Jonathan erfahren...
