Croatian surnames for male and females -Is there a differnece?

Not for points English to Croatian Social Sciences Names (personal, company)
I am told by a translator that the surname for our client's borther must have an 'a' added to the end of the name to indicate that it is her borther.
Our client disagrees and it's causing us some drama.
Can anyone clarify this for me?

Also, there could be a missunderstanding caused by a 'traditional' and/or 'local' ways of surname(s) declention ...
