Croatian surnames for male and females -Is there a differnece?

I am told by a translator that the surname for our client's borther must have an 'a' added to the end of the name to indicate that it is her borther.
Our client disagrees and it's causing us some drama.
Can anyone clarify this for me?

: in any case, the surnames do not change based on the sex of a person the way they do in, say, Czech...
