As simple a verb as "laittaa" might be, its meaning is 100% eluding me in this context.
If it means "to put/place", then *where* is the tracking form being placed?
This information is missing from the sentence entirely.
Or, is this a jargon-specialized meaning of "laittaa" that cannot be easily connected to the "put/place" meaning?

Please note it's not just "laitetaan", but "mukaan laitetaan". Laittaa mukaan means "to include; send toghether with" etc.
Of course it may mean fill out, too, (as Spencer said) - at least some fields in the form, but implicitly (not coming from "laatia" or anything). It's not clear from the context where it will be filled out, but I'd guess (as it is a tracking form) that each person would fill in their notes.

as in laatia? we need a Finnish speaker to confirm I think
