Severe mitral regurgitation is associated with progressive left ventricular dysfunction and congestive heart failure. Without intervention, symptomatic patients have an annual rate of death of 5% or more. Medical management alleviates symptoms but does not alter the progression of the disease.2 Current guidelines recommend surgery for moderate-to-severe (grade 3+) or severe (4+) mitral regurgitation in patients with symptoms or evidence of left ventricular dysfunction.

One surgical approach for mitral-valve repair involves approximation of the mitral leaflets with suture to create a double orifice. This procedure has been described for treatment of degenerative mitral regurgitation and is usually performed with an annuloplasty ring. Selected patients who have been treated with this technique as a stand-alone procedure have had successful results lasting up to 12 years.

A method for percutaneous double-orifice repair has been developed with the use of a mechanical device that is delivered into the left atrium through transseptal access. The device (MitraClip, Abbott Vascular) grasps and approximates the leaflets. Mitral repair with this device in 107 patients showed significant reduction in the severity of mitral regurgitation.

We designed the Endovascular Valve Edge-to-Edge Repair Study (EVEREST II), a randomized comparison of percutaneous mitral repair and mitral-valve surgery, to evaluate the efficacy and safety of percutaneous mitral-valve repair, as compared with conventional surgical repair or replacement.

Těžká mitrální regurgitace je spojena s progresivní dysfunkcí levé komory a městnavým srdečním selháním. Bez chirurgického zásahu je roční úmrtnost symptomatických pacientů 5 % nebo vyšší. Léčba mitrální regurgitace zmírňuje symptomy, nemá ovšem vliv na progresi nemoci. Podle současných směrnic jsou k operaci indikováni pacienti se středně těžkou (stupeň 3+) nebo těžkou (4+) mitrální regurgitací s příznaky dysfunkce levé komory srdeční.

Jeden z možných chirurgický postupů při plastice mitrální chlopně spočívá ve spojení cípů chlopně pomocí švu, čímž je vytvořeno dvojí ústí. Tento postup představuje chirurgické řešení degenerativní mitrální regurgitace. Zpravidla se při něm využívá anuloplastický prstenec. Léčba pacientů, kteří během studie podstoupili tuto proceduru jakožto izolovaný výkon, měla úspěšné výsledky s trváním až 12 let.

Při perkutánní intervenci za účelem vytvoření dvojího ústí se využívá mitrální svorka, která se zavádí do levé síně transseptálním přístupem. Mitrální svorka (MitraClip, Abbott Vascular) uchopí a přiblíží cípy chlopně. U 107 pacientů operovaných pomocí tohoto zařízení bylo pozorováno významné snížení závažnosti mitrální regurgitace.

Randomizovaná studie EVEREST II neboli studie endovaskulární záchovné operace mitrální chlopně typu „Edge-to-Edge“ byla provedena za účelem vyhodnocení účinnosti a bezpečnosti perkutánní plastiky mitrální chlopně ve srovnání s konvenční chirurgickou plastickou či náhradou chlopně.

Dění v jádru střední Evropy nelze pochopit bez širších souvislostí s minulostí či závislostí na ní. Proto autor zahajuje svůj výklad vznikem národních států po první světové válce, věnuje se jejich často dramatickému vývoji v meziválečném období i za druhé světové války, po níž se postupně všechny staly součástí sovětské sféry. Přestože byly osudy Maďarska, Československa a Polska v mnohém podobné, nevstoupily všechny tyto státy do transformace na začátku 90. let za stejných podmínek. V 70. letech se západní i východní blok přes všechny překážky obchodně sbližovaly – Západ v tehdejší ropné krizi potřeboval východní dodávky a Východ valutové zisky. Polsko a Maďarsko chtěly překonat stagnaci a oživit ekonomiku a zahájily první, ještě opatrné tržní reformy. Konzervativní režim v Československu k reformám nepřistoupil.
S rozpadem sovětského bloku ovládla osvobozené země představa návratu do (západní) Evropy, která byla neoliberální, a v neoliberalismu tudíž viděly svou budoucnost. Prvními kroky nových vlád bylo úsilí co nejrychleji se zbavit vazby na Sovětský svaz, klíčová byla likvidace vrcholných institucí – RVHP a Varšavské smlouvy. Polsko, Československo a Maďarsko se tak dostaly do bezpečnostního vakua, na což reagovaly vlastní iniciativou – jejich představitelé 15. února 1991 uzavřeli v maďarském Visegrádu dohodu, jejímž cílem byla koordinace úsilí o členství v Evropském hospodářském společenství; v tomtéž roce v Krakově dohodli i obdobnou součinnost v přípravách vstupu do NATO. Záměrem byl trvalý svazek, pro nějž se ujalo označení Visegrád podle místa prvního setkání.

What happens in the heart of Central Europe cannot be understood without a broader historical context and the region’s dependence on it. Therefore, the author begins with the emergence of nation states after the First World War, he deals with their often-dramatic development in the inter-war period and during the Second World War, after which they all gradually became a part of the Soviet sphere of influence. Although the fates of Hungary, Czechoslovakia and Poland were in many ways similar, these states didn’t start their transformation in the early 90s under the same conditions. In the 70s, the trade relations between Western and Eastern bloc were – despite all the obstacles – becoming tighter. With the oil crisis, the West needed supplies from the East and the East, reciprocally, foreign exchange profits. Poland and Hungary wanted to overcome stagnation and revitalize their economies, so they launched the first cautious market reforms. However, the conservative regime in Czechoslovakia did not introduce any reforms.
With the collapse of the Soviet bloc, the liberated countries conceived the idea of returning to (Western) Europe, which was neoliberal, because they saw their future in neoliberalism. The first step of the new governments was to get rid of the links to the Soviet Union as fast as possible, and the key was to destroy its supreme bodies – Comecon and the Warsaw Pact. Poland, Czechoslovakia and Hungary thus got into a security vacuum, to which they responded with their own initiative – on February 15, 1991 in Visegrád, Hungary, their leaders concluded an agreement with the aim to coordinate their efforts to join the European Economic Community; in the same year in Krakow they also agreed to cooperate in similar preparations for joining NATO. They intended to create a permanent union, which was later named after the place of their first meeting – the Visegrád group.
