Re: Osaid … D.o.B. …

Thank you for referring Osaid to CAMHS back in July 2014. Your letter states that Osaid’s parents had asked for support with his behaviour at home. I have since met with Osaid twice (3 and 30 September), spoken to his tutor Richard … on the telephone (30 September) and met with Osaid’s mum Alya … (8 December with an Arabic interpreter). I am now discharging Osaid from our service, partly as he turned 18 on 2 December but also because Osaid, his family and I do not see a clear role for mental health services.
Details of assessment
When I met with Osaid he did not present with any symptoms of mood disorder or significant mental illness. He was clearly very preoccupied with Inscape (school). He explained that going to school made him feel very anxious; he views the environment as too restrictive and wanted to be allowed to attend college. Unfortunately it seems that Osaid can be disruptive in class. He can also inadvertently put himself at risk in unsupervised environments due to his limited understanding of social rules and boundaries. Osaid has been known to attempt Tae Kwon Do moves in the community and may be at risk of approaching females. Osaid told me that he was keen to return to Stockport 6th form. He showed some understanding that making noises interrupted other students’ learning and said he would not do this again.
Mrs … feels confident about managing Osaid’s general behaviour at home. She keeps dangerous things out of his reach in case he throws them. When he is angry or distressed, she reassures him until he calms down. Mrs … explained that she and Inscape promised Osaid he would be able to go to college this year. As this hasn’t happened, Osaid has lost trust in his mum which is understandably upsetting for her. Osaid never wants to go to school in the morning. He says ‘I hate the place, it’s too restrictive, I’m not happy’. Mrs … responds by saying ‘this place is good for you’ – Osaid will then agree to go in. When he comes home he generally goes to his room and puts his headphones on. He seems restless, sitting for brief periods to eat or go on the Playstation then getting up and walking around. When Osaid asks for more independence Mrs … tries to compromise by letting him walk around a shop with his younger brother, keeping him in view at all times.
Summary and plan
Osaid desperately wants to be more independent and go to college like his peers. He is not interested in sleep hygiene or coping strategies for anger/anxiety and (according to his mum) only attended appointments because he believed I could get him into college. I can sympathise with Osaid’s position but I and Mrs … also recognise that Inscape provides a safe environment and may in fact be the best place available to him at the moment.
With regard to your original letter asking for support for Osaid’s parents, I have not spoken to Mr Catesh but I really could not add anything to Mrs ...’s current strategies. It was clear that she loves him very much and has developed a thoughtful and effective way of responding to his distress.
I made a referral to the Connexions Intensive Support Service in the hope that they could talk to Osaid about future options, or groups, available in Manchester. Unfortunately they have been unable to accept the referral as their funding now only covers young people who are not in employment education or training.
Mrs ... is uncertain about Osaid’s future at Inscape, for example whether he will ever be given another opportunity to attend college and what will happen when he is 19. I am sure she would appreciate a meeting with staff at Inscape and I wonder if you could arrange this?
I have agreed to forward Mrs ... the following:
- information about National Autistic Society groups in the local area
- a leaflet for Osaid’s younger brother to help him understand the diagnosis of ASD
- a small card with the CMFT logo on it stating that Osaid has a diagnosis of ASD that he can show to members of the public if needed
Osaid is now closed to Emerge. I have made Mrs ... aware that, if she has any concerns about his mental health in future, she should go to the GP to ask for a referral to adult mental health.

الأخ أسعد...تاريخ الميلاد...
شكرا الى أشارة أسعد بالعودة الى CAMHS في شهر 01/2014. رسالتك تشير الى أن والدا أسعد يحتاجون مساعدة بخصوص تصرفاته داخل المنزل. أنا منذ أن ألتقيته مرتين (التالث والتلاثون من شهر سبتمبر) وتكلمت مع المشرف الخاص به وأسمه ريتشارد...من خلال التلفون (30/09) حيث ألتقيت من والدة أسعد وأسمها علياء...في (08/09) وبوجود مترجم. أنا الأن سوف أستثني أسعد من خدماتنا, جزئيا هو أقترب من سن ال 18 في 02/09 لكن أيضا لأن أسعد وعائلته حيث أنني لا أري أي دور واضح لخدمات الصحة العقلية.
تفاصيل الموضوع:
عندا تقابلت مع أسعد لم تبدوا عليه أي أعراض لأضطراب المزاج أو مرض عقلي كبير. فهوكان واضحا أنشغال باله بأعمال المدرسة. هو أوضح لنا أن ذهابة الى المدرسة سوف يجعله يشعر قلق جدا, هو يعتقد أن البيئة تقييدية جدا ويريد أن يكون مسموحا له بالحضور في الكلية. لسوء الحظ يبدو أن أسعد من الممكن أن يكون تخريبي في الفصل. هو أيضا من الممكن ومن غير قصد أن يضع نفسة في خطر في بيئات لا تكون تحت الأشراف بسبب محدودية الفهم لدية من ناحية الأدوار الأجتماعية وحدودها. اسعد عرف عنه محاولة ......حركات في المجتمع ومن الممكن أن يشكل خطر بأستهدافه البنات أو الأناث. أسعد كان قد أخبرني من قبل أنه حاول العودة الى ستوك بورت (stockport) في يوم 06 . هو أظهر بعض الفهم أو المعرفة أن أفتعال الضجيج قد يعرقل دراسة الطلبة الأخرين و قال بأنه لن يفعل هدا مجددا.
السيدة..... تشعر بالثقة في أدارة السلوك العام لأسعد داخل المنزل. هي ملتزمة بأن تبقي الأشياء الخطيرة بعيدة عن متناوله خشية رميها عليهم. عندما يكون غاضي أو مضجور, هي كانت دائما تطمئنه حتى يهدأ. السيدة....أوضحت والمشرف أنها وعدت أسعد بأنه سوف يتمكن من الدخول الى لكلية هذه السنة. وبما أن هدا الشيئ لم يحدث, فأن أسعد قد فقد ثقته بأمه وهو أمر مفهوم بأنه مزعج بالنسبة لها. أسعد لا يريد أطلاقا الذهاب الى المدرسة في الصباح. هو يقول " أنا أكره هدا المكان", أنه تقييدي جدا, أنا لست سعيأ. الأنسة.....ردت عليه بأن قالت هذا المكان جيدا لك يا أسعد وسوف توافق بعدها الى الدهاب اليه. هو يبدوا أنه متجهم ويجلس لوهلة ليأكل أو ليلعب بالبلاستاشن ثم ينهض ويمشي. عندما أسعد يطلب أن يكون أكثر أستقلالية الأنسة..... تحاول أن توافقه بالسماح له بالمشي في المنطقة المحيطة والتسوق مع أخوه الأصغر وجعله تحت الملاحظة في كل الأوقات.
الخاتمة والخطة:
أسعد بحاجة ماسة يحتاج الى أن يكون أكتر أستقلالية ويحتاج للذهاب الى الكلية أسوة بأقرانه. هو غير مهتم بالنوم المنظم أو التعامل مع أستراتيجيات الغضب والقلق. حيث (وفقا لأمه) أنه فقط يحضر للمواعيد لأنه صدق بأنني بأستطاعتي أن أدخله للكلية. أنا بأستطاعتي أن أتعاطف مع وضع أسعد ولكن أنا و الأنسة ..... أيضا نعرف أن المدرسة توفر البيئة الأمنه و ربما في الحقيقة أنها أفضل مكان متوفر له في هده اللحظة.
فيما يتعلق بالرسالة الأصلية التي طلبتم فيها دعم أسامة ووالدية, أنا لم أتكلم مع الاخ كاتش (CATESH) ولكنني حقا أنا لم أستطع أن أضيف أي شيئ الى أستراتيجية الأنسة ..... أنه كان واضحا أنها تحبه كثيرا جدا وطورت طرق التفكير و التأثير لتتعامل مع محنته.
ان أرسلت أحالة الى خدمات الدعم مكثف الصلات على أمل أنهم يكونون قادرين على التحدث مع أسعد عن الخيارات المستقبلة والمجموعات المتوفرة في مانشستر (Manchester). لسوء الحظ هم أخبرونا أنهم غير قادرين على قبول هده الأحالة لأن أمكانياتهم المالية الان فقط تغطي الشباب الدين غير موظفين في التعليم أو التدريب.
الأنسة..... غير متأكدة من مستقبل أسعد مع المدرسة, على سبيل المثال سواء هو سوف يمنح فرصة أخرى لحضور للألتحق بالكلية و مادا سوف يحدث عندما يكون عمره 19 أنا متأكدة أنها سوف تثمن القاء مع الموظفين في المدرسة و أنا أتسأل لو أنتم بأستطاعتكم الترتيب لهدا اللقاء.
أنا كنت قد وافقت على تحويل الاتي الى الأنسة.....:
ـ الجمعية الوطنية للمصابين بالتوحد.
ـ كتاب الى الأخ الأصغر لأسعد لمساعدته على فهم وتشخيص ال ASD
ـ كرت صفير لأسعد مع CMFT ليوضح لعامة الناس أنه بصدد تلقي التشخيص المطلوب عن مرض التوحد.
أسعد الأن قريب من سن البلوغ. لقد جعلت الأنسة ..... على وعى أنه لو لديها أي قلق عن صحته العقلية في المستقبل, يجب عليها أن تدهب الى GP وتطلب الأحالة الى أخصائي الصحة العقلية .
