Ukrainian term

Ukrainian to English Other geneology
from Polish Parish Records....I think this is Ukrainian....I copied these three words from someones handwritten notes, so excuse spelling.

"[from the] book of records of the Office of Civil Registration [of such and such city or district]"

The first word seems to be transliterated "knigi", which means "book" in Genitive; ZAKS is probably ZAGS, which is a Soviet institution for civil registration; but the second word does not resemble what it should be ("zapysiv" in Ukr. or "zapisey" in Rus.), that is "records".
