Schweregradeinteilung

Discussion

MMUlr Jun 7, 2007:
Anne Zielisch MCIL (asker) Jun 7, 2007:

Anne Zielisch MCIL (asker) Jun 7, 2007:
Tut mir leid.
Anne Zielisch MCIL (asker) May 30, 2007:
das ganze Ding heisst: Schweregradeinteilung nach David S. Bell. Bei der Krankheit handelt es sich um das Chronische Erschöpfungssyndrom (Chronic fatigue syndrome).
Anne Zielisch MCIL (asker) May 30, 2007:
Das habe ich der Frau Ärztin auch gesagt... ...zuerst erhielt ich gar keinen Kontext und dann das oben Genannte. Ich werds noch mal versuchen.
MMUlr May 30, 2007:

Background: Chronic plaque-type psoriasis is a major dermatosis, but a significant question is still unanswered: What defines severity in chronic plaque-type psoriasis? While objective assessments like the Psoriasis Area and ** Severity Index ** (PASI) have frequently been used in clinical trials, quality of life (QOL) questionnaires are currently becoming more and more popular. Objective: This article summarizes the most important objective and subjective measurements of severity in psoriasis. For every dermatologist it is critically important to distinguish between severe psoriasis and psoriasis that severely affects QOL. Even if the PASI also has disadvantages, it is the most adequate instrument available to evaluate severity in plaque-type psoriasis. Result: We provide reasons why PASI >12 defines severe, PASI 7-12 moderate and PASI <7 mild chronic plaque-type psoriasis.

Introduction
Profound muscle fatigue, precipitated by minimal
physical activity, is one of the major symptoms in
** Chronic Fatigue Syndrome (CFS) ** patients [I]. In CFS
there have been reports of excessive intramuscular acidifi-
cation [2] and abnormal jitter with single fiber electro-
myography [3] suggestive of abnormal muscle membrane
function. However, other investigators have not found
abnormalities in muscle fatigability and excitattate oxidation has been reported to be reduced in CFS
patients [6] and intracellular concentration of ATP has
been demonstrated to be reduced at peak exercise in CFS
patients [7]. These results suggest a mitochondrial abnor-
mality.
Recent investigations in Great Britain have shown that
70% of CFS patients have ultrastructurally abnormal
muscle mitochondria [8-101. These mitochondria have
abnormal sizes, shapes and a peculiar infolding of the

degree of severity

Declined
Schweregrad = (degree of) severity (einer Krankheit)
(Langenscheidt Wörterbuch Medizin deutsch-englisch)

Rheumatic fever is a late inflammatory complication of acute group A streptococcal pharyngitis that manifests as an acute systemic febrile illness; it can include migratory arthritis involving the large joints, signs and symptoms of carditis and valvulitis, the erythema marginatum rash, subcutaneous nodules (Figure 1), and choreoathetotic movements of Sydenham's chorea. These problems can appear in different ***degrees of severity*** and in various combinations. The clinical episodes may be time limited, but damage to the heart valves may be chronic and progressive, resulting in significant cardiac dysfunction.

Diagnosis
Your doctor will ask about your symptoms and medical history, and perform a physical exam.
Like other burns, electrical burns have three ***degrees of severity***, each with distinctive symptoms:
First-degree burns injure only the outer layer of skin. They are red and painful, and may cause some swelling. The skin turns white when touched.
Second-degree burns are deeper and more severe. They cause blisters and the skin is very red or splotchy. There may be more significant swelling.
Third-degree burns cause damage to all layers of the skin down to the tissue underneath. The burned skin looks white or charred. These burns may cause little or no pain because the nerves in the skin are destroyed.

Here is an abstract written by David S. Bell himself:
Thirteen-Year Follow-Up of Children and Adolescents With Chronic Fatigue Syndrome
Received May 12, 2000; accepted Aug 25, 2000.
David S. Bell MD*, Karen Jordan, and Mary Robinson*
From * Primary Care Pediatrics, Lyndonville, New York; and the University of Illinois at Chicago, Chicago, Illinois.
Objective. To describe the educational, social, and symptomatic outcome of children and adolescents with chronic fatigue syndrome 13 years after illness onset.
Methods. Between January 1984 and December 1987, 46 children and adolescents developed an illness suggestive of chronic fatigue syndrome. Follow-up questionnaires were obtained from 35 participants an average of 13 years after illness onset. Data were obtained concerning subsequent medical diagnoses, amount of school missed, presence and ***severity*** of current symptoms, and subjective assessment of ***degree*** of illness resolution.
Results. Of the 35 participants, 24 were female (68.6%) and 11 were male (31.4%). Average age at illness onset was 12.1 years. Eight participants (22.9%) had an acute onset of symptoms, 27 (77.1%) had a gradual onset. No participant received an alternative medical diagnosis that could have explained the symptom complex between illness onset and follow-up. Thirteen participants (37.1%) considered themselves resolved of illness at follow-up; 15 participants (42.9%) considered themselves well but not resolved; 4 (11.4%) considered themselves chronically ill; and 3 (8.6%) considered themselves more ill than during the early years of illness. Correlation with the Medical Outcomes Study Short Form Health Survey was good for current level of symptoms and ***degree*** of recovery. Eight participants (22.9%) missed >2 years of school, and 5 of these were still ill at follow-up. Amount of school missed correlated with both illness severity at follow-up and perceived social impact of the illness.
Conclusions. These data demonstrate the presence of an illness consistent with the current definition of chronic fatigue syndrome. Eighty percent of children and adolescents affected had a satisfactory outcome from their fatiguing illness, although the majority of these participants had mild to moderate persisting symptoms. Twenty percent of participants remain ill with significant symptoms and activity limitation 13 years after illness onset. Chronic fatigue syndrome in children and adolescents may result in persistent somatic symptoms and disability in a minority of those affected. Key words: chronic fatigue syndrome, pediatric chronic fatigue syndrome.

This artice about Chronic Fatigue Syndrome also uses "degree of severity":

Chronic fatigue syndrome (CFS) is an emerging illness characterized by debilitating fatigue (experienced as exhaustion and extremely poor stamina), neurological problems, and a variety of flu-like symptoms. The illness is also known as chronic fatigue immune dysfunction syndrome (CFIDS), and outside of the USA is usually known as myalgic encephalomyelitis (ME). In the past the syndrome has been known as chronic Epstein-Barr virus (CEBV).
The core symptoms include excessive fatigue, general pain, mental fogginess, and often gastro-intestinal problems. Many other symptoms will also be present, however they will typically be different among different patients. These include: fatigue following stressful activities; headaches; sore throat; sleep disorder; abnormal temperature; and others.

The ***degree of severity*** can differ widely among patients, and will also vary over time for the same patient. Severity can vary between getting unusually fatigued following stressful events, to being totally bedridden and completely disabled. The symptoms will tend to wax and wane over time. This variation, in addition to the fact that the cause of the disease is not yet known, makes this illness difficult to diagnose.

Here is another article written by David S. Bell, using "degree of severity":
PROGNOSIS IN PEDIATRIC PATIENTS
Scientific reviews show that 54 to 94% of the pediatric CFS cases report full or significant partial recovery. Only a minority of children and adolescents remain severely disabled. However, the research issues in pediatric CFS are more complex than in adult, and at present debilitated.
Subsequently, no specific predictive factors are known. It is possible
that the ***degree of severity*** in the first few years of pediatric CFS may
predict likelihood of recovery. Once the diagnosis is made, alternative
explanations for disabling fatigue, such as diabetes or malignancy, are
rare.

Gemessen werden u.a. das activity level und die severity of illness:
"... severity of symptoms, the degree of activity impairment with both activity and rest, and the functional ability regarding full-time work"
