Thank you for the additional details, Anne. I came to the same conclusion as I progressed in the report, but receiving a confirmation of one's conclusion is always helpful.

@Susanna I am fairly sure that MMUlr is right. Your text seems to be an extract/printout from an electronic patient record. Entries to an electronic patient record are usually preceeded by a letter code indicating the type of entry (for example, A for Anamnese (history), B for Befund (physical findings), D for Diagnose (diagnosis), etc). I would not be surprised if these codes were either quite "abstract" per se in this particular practice software (?# for reason for presentation or (preliminary) diagnosis; o# for findings/tests), or they were special characters which were misread by the printer.

These special characters may be some kind of "tag" reflecting the bulleted paragraph style, due to the extraction of the text from file format, such as pdf, ppt ... etc.

If so, then you should ignore them.
