According to below references, "aushelfender Träger" is the local insurance carrier at the place of residence or place of stay of a person who has taken up the original insurance / is regularly insured overseas (with the so-called "zuständiger Träger" - the "competent insurer"). Most of the time, the term "institution at the place of stay" or something similar seems to be used, but there are examples for the use of "assisting institution" which I think fits better here seeing that "German" has already been specified in the sentence.
