The reference to Freud should help. This looks like it is from the 25th of his 28 lectures that make up the "Introductory Lectures on Psychoanalysis" from 1916.
There's a strong chance you'll be able to track this down somewhere.
Also, you need to be careful not to confuse "angoisse" and "anxiété". ;-)

Given the overall context, I think that it actually means "an imaginary situation", more along the lines of my initial interpretation ("fantasy"), because this chapter addresses emotions that are inhibited because of the fear that is felt in response to what are perceived to be dangerous situations. It has to do with the "fight or flight" approach when it comes to how one reacts to what seems to be a threatening event.

I think it seems to mean that the person concerned is constantly worried. A fantasy that is likely to be filled with just anguish?
