Cerfa form no. 11602*02 (an Institut National de la Propriété Industrielle form):

"BREVETS D'INVENTION, ***CCP***, TPS
MARQUES, DESSINS ET MODELES

DEMANDE D'INSCRIPTION D'UN ACTE AFFECTANT
LA PROPRIETE OU LA JOUISSANCE D'UN DEPOT"

I have found "certificats complémentaires de protection" for this, which I have for the moment as "additional protection certificates". Is there any UK/US equivalent? They are extensions once patents run out...

Thanks, great link. I have noted that they are roughly equivalent, as French legislation may differ a little from EU legislation, and I don't have time to look into it.
