Using your own hair-splitting logic where you say things could be interpreted wrongly (which is a legitimate approach), I really do feel that you should apply this to your own suggestion as well then:
"Persons with disabilities cared for by the taxpayer in their home"
Firstly, the taxpayer is NOT the caregiver so this should be "under care paid for by the taxpayer" (or something similar)
Secondly, "in their home" could be interpreted as "in the taxpayer's home" as the preceding noun is "the taxpayer" and not "the person cared for".
If you expect completely unambiguous translations from others, then you have to provide the same yourself.
At the end of the day, we just had to translate "PERSONNES RECUEILLIES HANDICAPÉES" and not add anything here as the rest is explained in the ensuing text.
