The two previous translations are more comprehensive than my suggestion. I am going for simplicity and flow.

Saarijärvi-Karstula Healthcare District; or
Saarijärvi and Karstula Healthcare District; or
Healthcare District of Saarijärvi and Karstula

It is a widely used term (just Google it), but it does ignore the "kuntayhtymä" completely, it depends on the document if it can be ignored.
