By inhibiting the 26S proteasome, bortezomib prevents its targeted proteolysis, thereby affecting multiple intracellular signalling cascades, including NF-ĸB (which promotes multiple myeloma cell growth, survival and drug resistance when activated in cellular stress), c-Jun N-terminal kinase (which promotes cell death in response to stress and elevated levels of misfolded proteins), caveolin-1 (which is involved in cell migration and motility), the cyclin-dependent kinase inhibitors p21 and p27, the proapoptotic proteins Bax and Bid and the tumour suppressor p53, and leading ultimately to cell death. As a result of its inhibition of NF-ĸB activation, bortezomib reduces the binding of multiple myeloma cells to bone marrow stromal cells "
