Nursing home - A residential facility for persons with chronic illness or disability, particularly older people who have mobility and eating problems. Also called a convalescent home, long-term care facility.
Residential (care) home - A care home is a residential setting where a number of older people live, usually in single rooms, and have access to on-site care services. A home registered simply as a care home will provide personal care only - help with washing, dressing and giving medication. Some care homes are registered to meet a specific care need, for example dementia or terminal illness.

Maybe you should define what is what and then there can be some translation of what each one does, as UK (I presume) and Slovakia realities in old folks care differs a lot. US and Czech differ so much, that any simple translation will be misleading.
