toire ikitai?
I teach at school as well so I know I should be down to earth, even though the answer doesn't sound polite.
If the child is very young, "oshikko?" is another alternative, though certainly not recommended to older people.

toire ni ikitaino?
Could be rephrased in a number of ways. This is a kind of way to address yourself to a child at around the kindergarten age. After saying this, you will guide him/her to the bathroom and tell how to use, etc...

toireni ikitai desuka? (トイレに行きたいですか?)
I think it is better to say "Do you want to go to the toilet?" since the person you are talking with is a kindergartener. That is how we say in Japan. (For translation, see above)
But if you want to know literal translation of your sentence, it will be, "toireni iku hitsuyou-ga arimasuka?" (トイレに行く必要がありますか?”

toire? or oshikko?
I've seen the proposed answers and all of them are good, but assuming that your mom knows litte or no Japanese, I wanted to make the phrase simple.
The meanings and pronunciations are
toire(bathroom): almost the same pronunciation as 'toilet', but omit the 't'sound at the end.
oshikko(pee pee): Oh-shit-koh
