I am currently working on a blood transfusion translation, and am having problems finding an appropriate English term for the above.

The context is as follows:

Om sociale druk bij donors uit te sluiten, krijgt elke donor een 'bloedbestemmingsformulier' waarop hij kan vermelden of hij al dan niet een risicogedrag vertoont. Het formulier is anoniem en heeft enkel een nummer, gelijk aan het donatienummer van het bloedzakje. Dit formulier steekt de donor in een speciaal daarvoor bestemde bus, die pas na de bloedinzameling wordt geopend door een bevoegde medewerker. De bloedgiften van donors die op het bloedbestemmingsformulier ‘JA’ aangeduid hebben worden onmiddellijk vernietigd.

'Blood designation/destination/purpose form' sounds a little contrived.
