Ich denke in Deinem Text bezieht sich die häusliche Pflege auf die Pflege der pflegebedürftigen Person selbst, d.h. Körperpflege etc., und die hauswirtschaftliche Versorgung auf die Bereiche der allgemeinen Versorgung, wie z.B. Einkaufen, Wohnungsversorgung, etc.
In diesem Fall wäre evtl. \"in-home patient care\" für die häusliche Pflege und \"home care assistance\" für die hauswirtschaftliche Versorgung angebracht.

: "home care assistance" sounds okay but is perhaps problematic: it doesn't cover e.g. shopping and could be read (out of context) as care in the home // You've convinced me, it obviously depends on country/continent!
