"Der Lokalbefund zeigte sich zunehmend in Demarkierung. Sich die Schwellung des Fusses war rückläufig."

"The local findings progressively appeared demarcated"... is what I can come up with without understanding what they mean by it (I don't like it).

Can someone explain and propose a suitable (idiomatic) translation into EN?

Thanks for the explanation, Anne. Unfortunately, the translation was delivered yesterday, but I did put ("shows progressive improvement") in brackets after "demarcation", so I think we're OK.

The patient may initially have suffered from infection of deeper (subcutaneous) tissue layers (= cellulitis). Skin redness in cellulitis tends to be poorly delineated. When the infection starts to heal, the inflammation recedes to the outer layers (= erysipelas) and then typically shows more distinct borders. This is a sign of healing – whereas "demarcation" is typically used to describe the establishment of a clear border in the development of necrosis, a process which indicates that the potential for healing is now lost. --> Although the same actually holds for "Demarkierung" in German, I would rather not use "demarcation" in your translation.
