Untersucht wurde die gesamte Lendenwirbelsäule von insgesamt BWK 11 bis zum Sakrum, *wobei als LWK 5 der Wirbelkörper kranial des letzten voll ausgebildeten Bandscheibenraums bezeichnet wird.*

My clumsy take is:
in which lumbar vertebral body 5 is designated with regard to the cranium as the vertebral body of the last fully formed intervertebral disk space.

Any better ideas?

The entire phrase: ...where L5 refers to the vertebral body occupying the position cranially to the last fully formed intervertebral disk space.

my possible suggestion: *whereby the craniovertebral junction (or body) at the last fully developed disc space is designated as LWK 5.*

Yes, I think you are on the right track - the last disc of the *Wirbelkorper kranial* is at the bottom junction, the way I understand it... but as I said, better to wait for answerers with more expertise! :)

Sorry - I am a bit confused how this covers the whole phrase
LWK = Lendenwirbelkörper = lumbar vertebral body with 5 being the lowest, close to the sacrum (https://www.spine-health.com/conditions/spine-anatomy/lumbar-vertebrae) whilst the craniovertebral junction is at the top of the spine (https://thejns.org/focus/view/journals/neurosurg-focus/38/4/article-pE2.xml (https://askdrtod.health/2020/05/17/the-craniovertebral-junction-the-most-important-place-you-never-heard-about/) "This is the brain body connection". Can also say intervertebral disc space but this is clear from the context.
