German Medical Medical (general) Medical history taken for

Autoimmunhepatitis mit Leberzirrhose, DJ-Dauerversorgung links bei Ureterstenose, arterielle Hypertonus, Bandscheidenvorfall [sic] zur *italienischen* und lumbal, hoher Blutdruck.

I am sure this must be an incorrect transcription or a typo - this text has a few of them (one report said the person had "Mehrfach auftretende progredienter Palpationen und Herzrasen." - I am pretty sure they meant "Palpitationen"). I cannot find an explanation anywhere.

Any ideas would be welcome...
