It's a certificate for the Fellowship of the Royal College of Physicians. I know what each word means, but put all together with their different cases, I keep arranging and rearranging. Everything I come up with is awkward.

(Jos is a city in Nigeria. In another certificate I found online, the city is declined in the dative.)

That explains it. I forgot the possibility of an I-stem. Thanks, everyone!

Edit: I just remembered my copy has "Florentissimo," which was also throwing off my translation, but since the one I found online has the feminine, I'll just assume it was a typo.

I would translate Jos as Locative. I agree with Sandra Mouton.

@Candace In that other certificate, the adjective is in the ablative (Londiniensis is like fortis, 3rd declension of parisyllabic adjectives), as is logical because it modifies Florentissima Academia.
And I suspect that Jos is left in an uninflected form because Nigerian names are less likely to have a Latin form.
