Your link refers to "CAROLINAS DE BACALHAU" (Carolinas in plural, not Carolina), but I have no idea if your actual text is in the singular. However, I am not 100% sure if "Carolina" or "Carolinas" (whatever your text actually states) refers to either North Carolina or South Carolina (USA), or both, but I think whatever translation you choose, it should include "Carolina" in the title, e.g. "Carolina-style cod (fish) cakes", Carolina codfish cakes, or some similar rendering.

Based on Beatriz' explanation (from Brazil) Carolinas were made with pâte à choux (a useful preparation like bechamel sauce), which is also used to make eclairs, for example. Specifically in the Asker's text, though, they are filled with a codfish (bacalhau) preparation. So, as the last link provided, I am inclined to use the word:

Question asked: Is there a US Equivalent? Is it a patty, or a Cream Puff, Carolina (PTBr as Beatriz suggests), or do you just describe the item and its ingredients with the use of a translator's note?

In this context, Carolinas have nothing to do with the U.S. Carolinas.
In Brazilian Portuguese food texts, Carolinas are a rather popular dessert filled with chocolate ganache or dulce de leche, for example.
Specifically in the Asker's text, though, they are filled withsome codfish, bacalhau, preparation.

Originally, Carolinas were made with pâte à choux (a useful preparation like bechamel sauce), which is also used to make eclairs, for example.
I believe today, many cooks have a recipe of their own and their Carolinas are just as good (especially when chocolate-filled and enjoyed by chocoholics like me!)

Thanks to a more comprehensive internet search by image, I could see pics of Cream Puffs from countries other than Brazil and USA.
Those not familiar with them will find a recipe and step-by-step pictures at, for example, https://www.tasteofhome.com/recipes/strawberry-cream-puffs/.
Personally, I have always ordered [pâte à choux] puffs with basis on the filling used.
My 1.5 cents to the Cooking / Culinary question re Brazilian Carolinas de Bacalhau.
