It pretty hard for me to translate this phrase in English.

The whole sentence is:
Im Zuge des intensivmedizinischen Aufenthaltes kam es bei bekanntem Vorhofflimmern und liegendem DDD-R Pacemaker zu tachykarden Episoden.

In German medical reports, "bei xy" often has the meaning of "with underlying xy". The wording "with underlying" does not seem particularly suitable in your case (I think), so you would have to re-word the sentence (e.g. according to lirka's suggestion).

The DDD-R pacemaker has been placed because the patient has cardiac rhythm disturbances due to atrial fibrillation. Within that background, despite the presence of the pacemaker, the patient still suffers from arrhythmia.
