"Esta arandela nos fija además el injerto y usaremos el extremo largo para reforzar la cápsula acromioclavicular, pasando el tendón por posterior o superior según consideremos más necesario, de forma habitual fijamos el tendón al extremo clavicular con sutura transósea y a cápsula en el lado acromial (Figura 28)."

From an article about shoulder grafts, and in particular a certain type of suture used for them. I'm completely confused by the wording of "Esta arandela nos fija además el injerto". Specifically "nos fija además" has got me utterly baffled. I'm not sure whether "además" means "in addition to" or "on top of". I know what "fijar" generally means, but its use with "nos" here is bizarre to me. Maybe it's some trick of the Spanish language that I don't know or have clean forgotten, but I just can't make head or tail of it. TIA for your help.

Arandela is basically a ring or washer.

"Nos fija" means it fixes/holds the implant or graft (or whatever) in place. It's usually done with clamps, which is why I'm surprised by the use of "arandela", but there are many methods, so there you go.

In my opinion, the use of "nos" (for us) is simply a communicative device to establish a sensation of closeness with the interlocutor or reader. This is commonplace in instructive or explicatory Spanish texts.

I'd say there is nothing odd or unusual about the use of "nos " in this text/situation.

Thanks for your input, but the primary issue for me here is not the meaning of "arandela", but the very strange (to me) usage of "nos fija". That's where I really need the help.
Thanks for the help about "nos". I should know that, really, but I haven't done any Spanish for a wee while and I'm a little rusty on its particular idiosyncrasies.
