Italian to English Bus/Financial Medical: Health Care
It's a letter issued by a new Sponsor of a clinical trial.

Would this be a power of attorney? letter of delegation?

I saw a few entries that support using "Letter of Delegation" but many more that say "Power of Attorney"

If other translators haven't come accross this, I might ask the CRO directly what they actually use!

It's authorization from AIFA to conduct a trial.

More specifically, authorization of a new trial Sponsor (CRO).

I saw this on another page:
9. Contract Research Organization: If sponsor has delegated authority to a “Contract Research Organization” (CRO), to coordinate research work with AUB, OGC requests a copy of the letter of delegation, which should include the agreed-upon activities and responsibilities of the CRO. The agreement should also specify the required quality and safety program provided by the CRO on behalf of the sponsor.

Letter of delegation

To confirm what you have already come up with, I regularly do translations for drug companies relating to clinical trials. This is my clients' preferred term. See also the second link which may be helpful in general.
