Ask the client But if they don't reply/have an answer, how about simply 'agreement'? Less specific but you avoid distortion and making assumptions.

Could you post some original language context? The sentence before/an example of a sentence in which your term appears generally helps.

What form does the "fiche" take? It could be a card, a sheet of paper, a slip of paper. "Fiche" is a bit of a catch-all term in French for which a more specific term is often necessary in English; card would not be accurate if this is in fact a piece of paper and vice versa.
