This is from an MHLW guideline on clinical trials.
This term is contrasted with 治験依頼者 (study sponsor), and obviously means a researcher who decides to initiate a clinical study themselves rather than at the request of a sponsor.
I am provisionally using 'independent initiator', and have also seen investigator-initiator, but I wonder if there is a standard term for what is obviously be a common concept.

Personally, I would go with "non-industry sponsored investigator" to make it clear or "independent investigator". If not "investigator initiator" as you suggested.

The term,「自ら治験を実施する者」 means a physicians or dentists plan who carry out investigator-initiated clinical trials.
My suggestion may be literal.

At an investigator-initiated clinical trial(医師主導治験), a physician is a sponsor & investigator, so we have often seen 'investigator-initiator'.
