: That's not an Anschreiben in German. Sorry, but no way. Anschreiben is a cover letter. If you go to http://wortschatz.uni-leipzig.de/ you'll see that it is impossible for Anschreiben to mean patient record or report.

The scenario I visualise is that the doctor has already sent out letters to a number of (all of?) his patients which included an error, such as dates when he is being replaced by a locum, or his surgery is closed during holidays or whatever. He is now correcting the error. Just a thought....
