From an engineering-based book from Spain for translation to British English, in this case with reference to ports in Spain around the year 1900. I just cannot think of the most appropriate adjective for cargo ports. There were also others of "categoría intermedia" and yet more "secundarios" at the same time:
"A finales de ese siglo, ... existirían tres puertos de primera categoría (Barcelona, Bilbao y Valencia), once de categoría intermedia (del nivel de Málaga o Almería) y noventa y cuatro secundarios."

otra posibilidad. Aunque literalmente se traduciría como puertos principales, creo que hace más referencia a la importancia de estos puertos dentro de la red de transportes marítimos. 'world class ports' me parece hacer más referencia a sus niveles de servicio y con 'international category ports' se enfoca un poquito más en los orígenes y destinos de los transportes.

: Yes, I don't think it's anything to do with the quality of service they provide or which destinations they serve (after all, a ship can go from anywhere to anywhere). It probably refers to the volumes of traffic they handle.
