What is the meaning of 元病変の拡大 in the following context?

審良正則先生には、HRCT所見をもとにした画像診断による鑑別についてご発表いただいた。先生は,初期悪化と薬剤性肺炎ではスリガラス影やconsolidationを認めるが、元病変の拡大があれば初期悪化が疑われること, 真の悪化では小葉中心性粒状影やtree-in-bud appearanceを里することを示された。

agree Carlis Hsu : yep, 元 refers to the `previous` one, but IMHO the use of such adjectives as primary, secondary, etc. is preferred to indicate the order of emergence in medical-related texts.

expansion of the original lesion/shade/ nodule

Here 元病変 is the original lesion/shade/ nodule.
元 means '元々ある’、'元々の' or ’元の大きさの' because this is just a finding of clinical imaging ,and primary lesion and previous one are not mentioned. Probably 'primary' I think, but it is not deterministic.
This patient had HRCT(ヘリカルCT), and ground-glass opacity and ground-glass opacity and consolidation were found.
This 元病変(an original nodule or nodule which was found originally or previously) is not always primary lesion and previous one (traces of pneumonia ,tuberculosis).
(Here we can't confirm if this 影 is primary lesion and previous one. If expansion in size of the original nodule, lesion or shade is found, initial aggravation would be suspected.
