Another option - but the report itself surely leaves no doubt
agree philgoddard : Lab is probably too informal though.

I've used the word "lab" with every lab report I have translated (excluding the name of the respective lab, where I do use "laboratory")
neutral British Diana : "Lab" would not be used in a title

I wouldn't use it for the name of a lab, but feel that it's fine for a header.
agree BrigitteHilgner : Make it "laboratory" and everyone should be happy.

agree Lirka : I believe 'lab report' is just fine. I think it does not need any additions, because it is evident from the context that they're not talking about a student's chemistry lab report, for instance...

agree Lirka : also possible, yes, but I don't see anything wrong with the simple 'lab report'

You're right, lirka. I just thought I'll try to include the '-ärztlich-' part of the source term in translation, so added the 'clinical' qualifier.

physician's report of laboratory findings

(Why not be literal? What is the compulsion to gloss over the disclosure that the author clearly intended?)
A "report of findings" is different from a "report", in that it discloses whether in the supervising physician's opinion the results were normal, and if abnormal the extent to which they were abnormal, and gives clinical conclusions about possible diagnoses, especially differential diagnoses. I don't deny that a "laboratory report" may contain these elements, but not to the extent of and with the authority of the supervising physician. That is the difference between a technician's report and a physician's report.

neutral Lirka : too convoluted; rarely used in reports

The task is to translate the source. I agree that typically no physician is actually involved in a routine lab report. Therefore it isn't called a "physician's report". But that's not what the source text says!

laboratory test results

Interpreting Laboratory Test Results

A laboratory test is a medical procedure in which a sample of blood, urine, or other tissues or substances in the body is checked for certain features. Such tests are often used as part of a routine checkup to identify possible changes in a person’s health before any symptoms appear. Laboratory tests also play an important role in diagnosis when a person has symptoms.

neutral Lirka : I wouldn't go with it b/c it implies 1 test only, whereas the DE text implies many findings...
