Some possibilities - all guesses Case Report, Clinical Record, Computed Radiography, Computed Radiology, Cardio-Respiratory, Controlled Respiration. My advice would be to translate the report first and then see which seems most appropriate.

Later on I have found "CR THORAX AAN BED". It is an X-ray report.
