The letters being people's names. I don't know whether CCC is a stand-in if someone falls ill, or whether he replaces BBB (and BBB alone) on alternate performances.

something about the fact that the name of the fourth actor appears in brackets immediately after the name of the third actor in a link to the same production (now removed) makes me think it means they alternate that role.
They don't normally mention general understudies in this sort of material, and a different word ('sostituto') is normally used for 'understudy', I believe.
