"La rappresentazione, affidata alla regia di XXX e alla direzione scenica di YYY, racconta l’avventura di un gruppo di amici, interpretati dagli attori ZZZ, AAA, BBB e, in alternanza, CCC".

The letters being people's names. I don't know whether CCC is a stand-in if someone falls ill, or whether he replaces BBB (and BBB alone) on alternate performances.

I think he is a stand-in for any of them.

in alternate performances

something about the fact that the name of the fourth actor appears in brackets immediately after the name of the third actor in a link to the same production (now removed) makes me think it means they alternate that role.
They don't normally mention general understudies in this sort of material, and a different word ('sostituto') is normally used for 'understudy', I believe.

I do not think this refers to the CCC actors being understudy.
Maybe the scene editing switches back and forth telling the stories of each respective friend: ZZZ, AAA, BBB and CCC.
