'volet' here means a type of form — I believe these are actually 2-part forms where one part is sent off to the Sécu, and the other part retained. But I don't think it is especially necessary or desirable in translation to go into details about the actual 'from' of this... form! Rather, I think it is more useful to talk about its 'function', and inasmuch as it is a type of 'declaration', I think 'return' is ideal here.
