I tend to agree with all previous answers, but one thing annoys me: what is the relation between patients' doctor visit and their death/myocardial infraction/CHF in one month and ten months?

I think their sufferings do not have anything to do with their visiting doctor, but rather to something being presented, i.e. introduced, to them. That something influences those of high BNP more than those of low BNP.

Coming not from medication practice background, I can only guess that something is drug or chemical substance. Regarding introduction method, there can be various ways: from (deliberate) injection to (accidental) contamination.

neutral Michael Barnett : Erich, "presentation" in this context is part of a specialized medical jargon. Brie is 100% correct. The death is not caused by the doctor's visit, but the date marks a precise time for statistical purposes. Presumably, the pt "presented" due to symptoms

Presentation=doctor visit is easily accessible via online dicos. But, supposed I have hi-BNP and visit a doctor this morning; am I to be dead 1 or 10 months later? There must be more effort to explain. Otherwise, I'll cancel my doctor appointment... ;)
