: I think you have misunderstood the definition. A second line therapy would be used initially if the pt were allergic to the first line drug. If he were too young for the 2nd line drug, a 3rd line drug would be used initially.

"First line therapy" is a term that is applied to a broad range of subjects in medicine, not just cancer therapy.
Penicillin, for instance, is first line therapy for "strep throat" because the microbe is virtually universally sensitive to this medication. However, patients who are known to be allergic to penicillin are given "second line" therapy, usually erythromycin. If the patient happens also to be allergic to erythromycin, he will be offered a "third line" treatment. Obviously, if the patient is known to be allergic beforehand, he is not going to be offered penicillin initially, unless, of course, the physician wishes to be sued for malpractice.
Similarly, first-line treatments are sometimes not prescribed initially because the drug may not be suitable for patients under 18 years of age, or perhaps there is a drug incompatibility with another medication that the patient is taking or perhaps the patient may simply not be able to afford the "first line" drug.
So, while the "first line" therapy is usually the initial treatment used, it can be seen from a careful reading of my reference that it is "first line" not becauase it is used initially, but because it has been determined from clinical trials to be the best therapy for a particular disorder.

: The article you offer as a reference, doesn't talk about "2nd or 3rd line" used initially. Alternative therapy is used (alone or with the 1st ln th) if (i.e. AFTER it's determined that) the 1st line is failing or producing adverse side effects
