Since it's a psychiatric report, it is, in my opinion, important to mention the psychological state of the patient. This term is used in psychiatry more often than "certificat de bonne santé".

The actual idiom "give someone a clean bill of health" would be translated as "trouver quelqu'un en parfait état de santé" (lit. "to find someone in a pefect state of health" or "to find someone to be perfectly healthy.").

According to Le grand dictionnaire terminologique (www.granddictionnaire.com) bill of health is patente de santé in French, but it doesn't seem to fit in your context. I would use sth with "Déclaration d'état de santé" instead.
