could mean that it spreads across a large area...

Erysipelas is an acute bacterial infection of the skin and subcutaneous tissues ... Severe cases where there is an extensive involvement of the skin...

"Flächig" means that the rash/the infection involves a continuous area of the skin (rather than spots as, e.g., in impetigo). IMO, this is mostly described as "spreading skin infection".

Thank you, MM! There certainly is a *spread* of infection within or between individuals, but if this was the only meaning of "spreading", impetigo rather than cellulitis should be called a "spreading skin infection"!

Patchy= fleckig/flächig, wenn die Hautinfektion eher großflächig ist, würde ich mich allerdings eher für "extensive" entscheiden, wie von Lydia schon vorgeschlagen
