"Flächig" means that the rash/the infection involves a continuous area of the skin (rather than spots as, e.g., in impetigo). IMO, this is mostly described as "spreading skin infection".

Thank you, MM! There certainly is a *spread* of infection within or between individuals, but if this was the only meaning of "spreading", impetigo rather than cellulitis should be called a "spreading skin infection"!

Patchy= fleckig/flächig, wenn die Hautinfektion eher großflächig ist, würde ich mich allerdings eher für "extensive" entscheiden, wie von Lydia schon vorgeschlagen
