What is Sports medicine?
Sports medicine is a branch of medicine that deals specifically with the diagnosis, treatment and prevention of injuries or illness resulting from sports, exercise or recreational activities. It also involves sports training which includes exercise and workouts, sports psychology and sports nutrition. Sports medicine may be a specialization of an orthopedic surgeon; however, sports medicine is not restricted to one profession.
The orthopedic sports medicine is an area under the sports medicine that focuses on identifying, preserving and restoring affected body structures of the musculoskeletal system via medical, surgical and rehabilitative approach.
A sports medicine specialist is an individual who receives specialized education and get the necessary training that focus on the medical and therapeutic side of sports and other physical and recreational activities. The specialist is not a physician. He or she could come from different disciplines or profession.
A sports medicine physician is a medical doctor who gets a specialized training in sports medicine, which is a 1-2 years of additional training through an accredited subspecialty program. The sports medicine physician is a leader of the sports medicine team. The team would include physicians and surgeons, physical therapists, coaches, athletic trainers, parents, personnel and the athletes.
The sports medicine team may appoint a Certified Athletic Trainer (ATC) whose responsibilities includes providing first aid care, initiating treatment procedure, design and implementation of rehabilitative protocol, inventory and acquisition of supplies, applying techniques for athletes recuperation, and completion of medical and accidental forms.
The common sports medicine injuries are divided into two categories: acute and chronic. Acute injuries are those that are resulting from a sudden fall, collision, or a twist. Chronic injuries are those that results from an overuse of a body part in a repetitive stress. Some of these common sports injuries are ankle sprain, hamstring strain, knee injury, tennis elbow and shoulder injury.
Sports medicine specializes in preventing, diagnosing and treating injuries related to participating in sports and/or exercise, specifically the rotation or deformation of joints or muscles caused by engaging in such physical activities. The sports
medicine “team” includes specialty physicians and surgeons, athletic trainers, physical therapists, coaches, other personnel, as well as the athlete himself/herself. Because of the competitive nature of sports, a primary focus of sports medicine is the rapid recovery of patients, which drives many innovations in the field.
Sports medicine has always been difficult to define because it is not a single specialty, but an area that involves health care professionals, researchers and educators from a wide variety of disciplines. Its function is not only curative and rehabilitative, but especially preventive.
Despite this wide scope, there has been a tendency for many to assume that sport-related problems are by default musculoskeletal and that sports medicine is an orthopaedic specialty. There is much more to sports medicine than just musculoskeletal diagnosis and treatment. Illness or injury in sport can be caused by many factors from environmental to physiological and psychological. Consequently, sports medicine can encompass an array of specialties, including cardiology, pulmonology, rehabilitation medicine, orthopaedic surgery, nutrition, dentistry, opthamology, exercise physiology, biomechanics, and traumatology. For example, heat, cold or altitude during training and competition can alter performance or may even be life threatening. The female athlete triad of disordered eating, menstrual disturbances, and bone density problems, and the problems of pregnant or aging athletes demand knowledge from many diverse fields. In addition, the management of endocrinological diseases and other such problems in the athlete demands both medical expertise and sport-specific knowledge.
What is the history behind HBOT?
In the early 1900s, Orville Cunningham noticed that people with some heart diseases did better if they lived closer to sea level than at high altitudes. He successfully treated a colleague with influenza who was near death due to lung restriction, and later developed a hyperbaric chamber. After his attempts to use HBOT to treat a host of other conditions failed, the method was abandoned and his chamber was scrapped.
HBOT chambers were developed by the military in the 1940s to treat deep-sea divers who suffered from decompression sickness. In the 1950s, HBOT was first used during heart and lung surgery. In the 1960s, HBOT was used for carbon monoxide poisoning, and it has since been studied and used for a number of health-related applications. It has been the subject of a great deal of controversy
because of the lack of scientific proof to support many of the other uses for which it is suggested.
In recent years, professional and college teams have started using hyperbaric oxygen therapy (HBOT) to treat sports injuries. From muscle contusions and ankle sprains to delayed-onset muscle soreness, HBOT has been used to facilitate soft-tissue healing . To minimize the time between injury and HBOT treatment, some professional sports teams have on-site centers. Because of the importance of oxygen in the aerobic energy system, many athletes and researchers have also investigated the possible ergogenic effects of HBOT. Hyperbaric oxygen (HBOT) is used in a sports medicine setting to reduce hypoxia and edema and appears to be particularly effective for treating crush injuries and acute traumatic peripheral ischemias. When used clinically, HBOT should be considered as an adjunctive therapy as soon as possible after injury diagnosis. During HBOT treatment, a patient breathes 95% to 100% oxygen at pressures above 1.0 atmosphere absolute (ATA). Normally, 97% of the oxygen delivered to body tissues is bound to hemoglobin, while only 3% is dissolved in the plasma. At sea level, barometric pressure is 1 ATA, or 760 mm Hg, and the partial pressure of oxygen in arterial blood (PaO2) is approximately 100 mm Hg. At rest, the tissues of the body consume about 5 mL of O2 per 100 mL of blood. During HBOT treatments, barometric pressures are usually limited to 3 ATA or lower. The oxygen content of inspired air in the chamber is typically 95% to 100%.
The combination of increased pressure (3 ATA) and increased oxygen concentration (100%) dissolves enough oxygen in the plasma alone to sustain life in a resting state. Under hyperbaric conditions, oxygen content in the plasma is increased from 0.3 to 6.6 mL per 100 mL of blood with no change in oxygen transport via hemoglobin. HBOT at 3.0 ATA increases oxygen delivery to the tissues from 20.0 to 26.7 mL of O2 per 100 mL of blood.
Proposed Healing Mechanisms
Increased oxygen delivery to the tissues is believed to facilitate healing through a number of mechanisms.
Vasoconstriction
High tissue oxygen concentrations cause blood vessels to constrict, which can lead to a 20% decrease in regional blood flow. In normoxic environments, tissue hypoxia may develop; however, this is not the case with HBOT. The increased plasma oxygen that reaches the tissue more than compensates for the decrease in regional blood flow.
The net effect is decreased tissue inflammation without hypoxia--a mechanism by which hyperbaric oxygen therapy is believed to improve crush injuries, thermal burns, and compartment syndrome.
Neovascularization and epithelialization
High tissue oxygen concentrations accelerate the development of new blood vessels. This can be induced in both acute and chronic injuries. Regenerating epithelial cells also function more effectively in a high-oxygen environment. These effects have proven effective in treating tissue ulcers and skin grafts.
Stimulation of fibroblasts and osteoclasts
In a hypoxic milieu, fibroblasts are unable to synthesize collagen, and osteoclasts are unable to lay down new bone. Collagen deposition, wound strength, and the rate of wound healing are affected by the amount of available oxygen. Ischemic areas of wounds benefit most from the increased delivery of oxygen. HBOT increases tissue levels of oxygen, allowing for fibroblasts and osteoclasts to function appropriately. This mechanism may play a role in the treatment of osteomyelitis and slowly healing fractures.
Immune response
When tissue oxygen tensions fall below 30 mm Hg, host responses to infection and ischemia are compromised. Studies have shown that the local tissue resistance to infection is directly related to the level of oxygen found in the tissue. High oxygen concentrations may prevent the production of certain bacterial toxins and may kill certain anaerobic organisms such as Clostridium perfringens.
More important, however, oxygen aids polymorphonuclear leukocytes (PMN). Oxygen is believed to aid the migration and phagocytic function of the PMN. Oxygen is converted within the PMN into toxic substrates (superoxides, peroxides, and hydroxyl radicals) that are lethal to bacteria. These effects on the immune system allow HBOT to aid the healing of soft-tissue infections and osteomyelitis. HBOT has also been found to inhibit PMN adherence on postcapillary venules. Although this may seem paradoxic, this effect is beneficial because it helps limit reperfusion injury after crush injury and compartment syndrome.
Maintaining high-energy phosphate bonds
When circulation to a wound is compromised, resultant ischemia lowers the concentration of adenosine triphosphate (ATP) and increases lactic acid levels. ATP is necessary for ion and molecular transport across cell membranes and maintainance of cellular viability. Increased
oxygen delivery to the tissue with HBOT may prevent tissue damage by decreasing the tissue lactic acid level and helping maintain the ATP level. This may help prevent tissue damage in ischemic wounds and reperfusion injuries. HBOT is an effective treatment for crush injuries and other acute traumatic peripheral ischemias because it alleviates hypoxia and reduces edema; however, clinical experience with HBOT for sports injuries is limited. Also, the criteria for using HBO2 in acute traumatic peripheral ischemias are not clearly established. HBOT should be considered as an adjunctive therapy as soon as possible after injury diagnosis. Treatment pressures for acute traumatic peripheral ischemia range from 2.0 to 2.5 ATA, with a minimum of 90 minutes for each treatment.
HBOT has been used to treat joint, muscle, ligament, and tendon injuries in soccer players in Scotland. When HBOT was used in conjunction with physiotherapy, the time to recovery was reduced by 70%. The results compared a physiotherapist's estimation of the time course for the injury and the actual number of training days missed. The absence of a control group and objective measures to assess the injury weaken the encouraging findings in this study. HBOT has been used to treat acute ankle injuries. Borromeo et al conducted a randomized double-blind study of 32 patients who had acute ankle sprains to compare HBOT treatment at 2.0 ATA with a placebo treatment. Each group received three treatments: one for 90 minutes and two for 60 minutes. The improvement in joint function was greater in the HBOT group compared with the placebo group. There were no statistically significant differences between the groups when assessed for subjective pain, edema, passive or active range of motion, or time to recovery. Study limitations included an average delay of 34 hours from the time of injury to diagnosis, administration of only three treatments within 7 days, treatment pressure of only 2.0 ATA, and short treatment duration.

تمرین و یا فعالیتهای تفریحی در ارتباط است. این رشته همچنین شامل ورزش و تمرین و روانشناسی ورزشی و تغذیه
ورزشی می شود. طب ورزشی می تواند تخصص در جراحی ارتوپدیک باشد، با این حال طب ورزشی به یک حرفه
محدود نمی شود.

تاریخچه HBOT یا درمان با اکسیژن پر فشار:
در اوایل قرن بیستم، اورویل کانینگهام متوجه شد که افرادی که دارای بیماری قلبی هستند در صورتیکه هم سطح دریا
زندگی کنند تا در ارتفاعات، زندگی بهتری خواهند داشت. او با موفقیت توانست یکی از همکارانش که دارای بیماری
آنفولانزا بود و در اثر انسداد ریه در حال مرگ بود را درمان کند و پس از آن اتاق پرفشار را ساخت. پس از تلاشهای او
برای استفاده از درمان با اکسیژن پرفشار برای درمان، تعدادی از شرایط با شکست مواجه شد و این روش کنار گذاشته
شد و اتاقش هم لغو شد. اتاقهای HBOT در دهه 04 توسط ارتش برای درمان غواصان دریاهای عمیق که دچار بیماری
، رفع فشار می شدند، ساخته شد. در دهه 04 HBOT برای اولین بار در عمل جراحی قلب و ریه مورد استفاده قرار
گرفت. در دهه 04 ، برای مسمومیت با مونو اکسید کربن مورد استفاده قرار گرفت و از آن زمان مورد مطالعه قرار گرفته
است و در بسیاری از کاربردهای مرتبط با قلب مورد استفاده قرار گرفته است. البته به دلیل عدم اثبات علمی برای حمایت
از بسیاری از کاربردهای پیشنهادی دیگر مورد مناقشه قرار گرفته است.
در سالهای اخیر، تیمهای دانشگاهی و حرفه ای استفاده از درمان با اکسیژن پرفشار برای درمان صدمات ورزشی آغاز
کرده اند. از کشیدگی ماهیچه و پیچ خوردگی مچ پا گرفته تا تاخیر در شروع درد عضلانی. درمان با اکسیژن پرفشار
برای تسهیل در بهبود بافت نرم به کار برده شده است. برخی از تیمهای پزشکی برای به حداقل رساندن زمان بین آسیب

در محیطهای نورموکسیک، امکان به وجود آمدن کمبود اکسیژن وجود دارد اما این مساله در HBOT وجود ندارد.
اکسیژن افزاینده پلاسما که به جای جبران کاهش جریان خون منطقه ای به بافت می رسد. تاثیر شبکه ای، کاهش تورم
بافتی بدون کاهش اکسیژن در بافتهای بدن است، مکانیسمی که درمان با اکسیژن پرفشار به وسیله آن صدمات له شدگی،
سوختگی، و سندرومهای جزیی را درمان می کند.
نئوواسکولاریزاسیون و اپیتلیازیون
غلظتهای بالای اکسیژن بافتی ساخت شریانهای جدید خونی را افزایش می دهد. این کار می تواند هم در صدمات حاد و
مزمن کاهش بیابد. بازسازی سلولهای اپیتلیال همچنین در یک محیط با اکسیژن بالا کاربرد دارد. این تاثیرات در درمان
بافت زخمی و پیوند پوست موثر هستند.
تحریک فیبروبلاستها و اوستئوکلاستها
درمحیط هیپوکسیک )کمبود اکسیژن در بافت( ، فیبروبلاستها قادر به سنتز کلاژن نیستند و اوستئوکلاستها قادر به ساختن
استخوان جدید نیستند. رسوب کلاژن، تقویت زخم و میزان بهبودی زخم تحت تاثیر میزان اکسیژن موجود است.
مناطق اسمکیک زخمها بیشتر از دریافت افزاینده اکسیژن سود میبرند. HBOT میزان اکسیژن بافتی را افزایش می دهد
و به فیبروبلاستها و اوستئوکلاستها اجازه فعالیت مناسب را می دهد. این مکانیسم میتواند در درمان استئوملیتیس نقش
داشته باشد و شکستگیها را به آرامی بهبود دهد.
پاسخ ایمنی
وقتی فشارهای اکسیژن بافتی به کمتر از 34 میلی لیتر افت می کند، بافت میزبان به عفونت پاسخ می دهد و ایسکمیک
بوجود می آید. مطالعات نشان داده است که مقاومت بافت محلی نسبت به عفونت رابطه ای مستقیم با سطح اکسیژن موجود
در بافت دارد. غلظتهای بالای اکسیژن می تواند مانع تولید بعضی از سمهای باکتریایی شود و میتواند بعضی از
ارگانیسمهای بی هوازی مثل کلستریدیوم شود.
مهمتر از این، اکسیژن به لکوسیت پلی مورفونوکلر (PMN) کمک می کند. به نظر می رسد که اکسیژن به حرکت و
عملکرد فاگوسیتیک PMN کمک میکند. اکسیژن در PMN به لایه های سمی )سوپراکسید، پراکسید، رادیکالهای
هیدروکسیل( تبدیل می شود که برای باکتری ها کشنده هستند. این تاثیرات بر سیستم ایمنی به HBOT اجازه می دهد تا
به بهبود عفونت و اوستئومیلیتیس بافت نرم کمک کند. HBOT همچنین در مهارچسبندگی PMN در پستکاپیلری یافت
شده است. اگرچه این کار متناقض به نظر می رسد اما این تاثیر سودمند است چون به محدود کردن جریان خون در قسمت
آسیب دیده و سندروم جزیی کمک می کند.
حفظ اتصالات فسفات با انرژی بالا
هنگامیکه گردش خون در یک زخم اتفاق می افتد، ایسکمیک بوجود آمده غلظت تری فسفات آندوسین (ATP) را کاهش
داده و میزان اسید لاکتیک را افزایش می دهد. ATP برای حمل و نقل آیون و مولکولی درغشاء سلولی و حفظ زیست
سلولی لازم است.
افزایش تحویل اکسیژن به بافت بوسیله HBOT می تواند مانع آسیب بافت بوسیله کاهش میزان اسید لاکتیک بافت شود و
به حفظ میزان ATP کمک می کند. این کار می تواند مانع آسیب بافتی در جراحات ایسکمیک و آسیبهای مجدد رفیوژن
شود. HBOT یک درمان موثر برای صدمات له شدگی و و سایر آسیبهای حاد محیطی است چون هیپوکسی و التهاب را
ماهش می دهد، با این حال تجربه بالینی برای استفاده از HBOT یا درمان با اکسیژن پرفشار محدود است. همچنین
معیار استفاده از HBO2 در ایسکمیک محیطی ترائومیک حاد به وضوح مشخص نشده است. HBOT باید به عنوان
یک درمان کمکی و جانبی بلافاصله پس از تشخیص آسیب در نظر گرفته شود. فشارهای درمانی برای ایسکمیک محیطی
ترائومیک حاد از 2.4 تا 2.0 ATA با حداقل 54 دقیقه برای هر درمان متغیر است.
HBOT برای درمان مفصل، عضله، رباط و تاندون در بازیهای فوتبال اسکاتلند به کار برده شده است. هنگامیکه
HBOT در کنار فیزیوتراپی به کار برده شد. زمان بهبودی تا 94 % کاهش یافت. نتایج با تخمین زمان فیزیوتراپیست
برای صدمه و عدد واقعی روزهای از دست رفته تمرین و ورزش مقایسه شد. فقدان گروه کنترل و اقدامات عینی برای
ارزیابی آسیب نتایج تشویق کننده مطالعه را ضعیف نمود. HBOT برای درمان صدمات حاد مچ پا به کار برده می شود.
برومئو و سایرین یک مطالعه دو سویه را بر روی 32 بیمار که دارای پیچ خوردگی حاد مچ پا بودند انجام دادند تا درمان
با اکسیژن پرفشار در ATA2.0 با درمان دارویی مقایسه کنند. هر گروه سه دوره درمانی را دریافت نمود. یک دوره 54
دقیقه ای، دو دوره 04 دقیقه ای. بهبودی در مفصل در گروه HBOT در مقایسه با گروه دارویی بهتر بود. هیچ تفاوت
معنادار آماری بین دو گروه در ارزیابی درد، التهاب و میزان فعال یا غیر فعال یا زمان بهبودی وجود نداشت. محدودیتهای
مطالعه شامل میانگین تاخیر 30 ساعت از زمان آسیب تا تشخیص، انجام تنها 3 درمان ئر طول 9 روز، فشار درمان
ATA2.0 و مدت زمان کوتاه درمان بود.

درباره سفر از یک جنگ و نا امنی تا امنیت در نروژ که فضا برای موقعیتهای
جدید را فراهم می کند، صحبت کند.

خانواده که اینچنین راحت نقل می شود هم آموزنده و هم حیرت انگیز است،
