Source text - French Introduction : Depuis maintenant près d'im siècle, l'utilisation des agrafes en chirurgie s'est imposée en apportant une sécurité et une fiabilité lors de la confection d'une anastomose. Plusieurs études ont montré une équivalence entre anastomose manuelle et anastomose mécanique. Cependant il n'existe pas de recommandations sur les conditions d'utilisation des agrafes en fonction de la qualité et de l'épaisseur du tissu à agrafer. Un bon agrafage correspond à un équilibre entre un bon rapprochement des tissus et ime hémostase correcte sans entraîner d'ischémie et de déchirure. Grâce à la cœlioscopie, la chirurgie bariatrique a pris son essor depuis une vingtaine d'année. Les différentes techniques modernes (sleeve gastrectomie, By pass gastrique, Switch duodenal) ont recours dans la plupart des cas à un agrafage mécanique des différentes parties du tube digestif (estomac, jéjunmn, iléon). Peu d'études ont permis de dégager des recommandations sur l'utilisation des agrafeuses en chirurgie digestive. Mery et als ont analysé les différentes hauteurs d'agrafes ainsi que le chevauchement des lignes d'agrafes sur la survenue d'une fistule sur l'intestin grêle. La complication la plus redouté en chirurgie viscérale est la déhiscence anastomotique. Le taux de fistule est de 0.3% à 4.2% pour le By-pass gastrique, de 0 à 5% pour la sleeve gastrectomie et de 2.5 à 19 % en chirurgie colorectale. Une meilleure compréhension des phénomènes de compressions et des systèmes de résistances aux pressions de l'agrafage mécanique est nécessaire pour minimiser le risque de fistules anastomotiques. Objectif principal : L'objectif principal de l'étude est l'effet de la hauteur d'agrafes sur la survenue d'une fistule lors d'une sleeve gastrectomie chez im modèle porcin. Objectifs secondaires : - Evaluer le site de survenue d'une fistule sur la ligne d'agrafes d'une sleeve gastrectomie - Evaluer l'effet du chevauchement sur la ligne d'agrafes sur la survenue d'ime fistule lors d'une sleeve-gastrectomie chez im modèle porcin. Population Un modèle porcin est utilisé pour réaliser ces expériences. Les animaux ont subi des expériences en chirurgie colorectale ou en chirurgie thoracique (sans chirurgie oesophagienne). On attache l'œsophage distal, avec un drain thoracique. La ligne d'agrafes avec les caractéristiques souhaitées a été créée sur le tube gastrique. On immerge tout l'échantillon dans un récipient rempli d'eau. L'air est ensuite lentement introduit dans l'œsophage distal par l'intermédiaire du drain thoracique et une poire d'un manomètre. La pression dans le système a été suivi continu avec un manomètre (Marque à définir).	Translation - English Introduction: For nearly a century, the use of staples in surgery has proved to be essential, providing safety and reliability during anastomosis procedures. Several studies have shown no difference between manual anastomosis and mechanical anastomosis. However, there are no recommendations on the conditions of use for staples in terms of the quality and thickness of the tissue to be stapled. Good stapling is a balance between good closure of the tissues and good hemostasis without causing ischemia or tearing. Thanks to laparoscopy, bariatric surgery (surgery of obesity) has boomed over the last 20 years. In most cases, the different modern techniques (sleeve gastrectomy, gastric bypass surgery and duodenal switch surgery) use mechanical stapling for the different parts of the digestive tract (stomach, jejunum and ileum). Only a few studies have been performed that allow us to provide recommendations on the use of staplers in digestive surgery. Mery et al. analyzed the different staple heights as well as the overlapping of the staple lines on the occurrence of fistulas in the small intestine. Anastomotic dehiscence is the most feared complication during visceral surgery. The fistula rate is from 0.3% to 4.2% for gastric bypass surgery, 0 - 5% for sleeve gastrectomy and 2.5 - 19% for colorectal surgery. A better understanding of the compression phenomena and pressure resistance systems of mechanical stapling is necessary to minimize the risk of anastomotic fistulas. Primary Objective: - The primary objective of this study is to study the effect of staple height on the occurrence of fistulas during sleeve gastrectomy in a porcine model. Secondary Objectives: - To evaluate the site of occurrence of fistulas on the staple line of sleeve gastrectomy surgery; - To evaluate the effect of overlap at the staple line on the occurrence of fistulas during sleeve gastrectomy surgery in a porcine model. Population A porcine model was used to perform these experiments. The animals underwent experiments in colorectal surgery or thoracic surgery (with no esophageal surgery). The distal esophagus is attached with a thoracic drain. The staple lines with the desired characteristics were created in the gastric tube. The whole sample is submerged in a container filled with water. Air is then slowly introduced into the distal esophagus through the use of a thoracic drain and a hand bulb from a pressure gauge. The pressure in the system was monitored continuously with a pressure gauge (brand to be confirmed).
