Tyrosinemia type 2 is a genetic disorder in which individuals have elevated blood levels of the amino acid tyrosine, a building block of most proteins. This condition can affect the eyes, skin, and intellectual development. Symptoms of tyrosinemia type 2 often begin in early childhood and include excessive tearing, abnormal sensitivity to light (photophobia), eye pain and redness, and painful skin lesions on the palms and soles (palmoplantar hyperkeratosis). About 50 percent of individuals with this condition have an intellectual disability. Tyrosinemia type 2 is caused by a deficiency of the enzyme tyrosine aminotransferase, one of the enzymes required for the multi-step process that breaks down tyrosine. This enzyme shortage is caused by mutations in the TAT gene. This condition is inherited in an autosomal recessive manner.u There is no cure for this condition; however, some of the symptoms may be managed with a diet that limits certain amino acids, such as phenylalanine andyrosine. A medication called NTBC may also be used to help control the amount of tyrosine in the body.

Mr. XXX, 63 year old XXX gentleman with no significant past medical history was seen in XXX hospital in May 2015 with complaints of back pain of 1 V2 months duration. MRI spine revealed multiple disc lesions and osteopenia. He was referred to XXX hospital where clinical examination revealed anemia and investigations showed Hb 87g/l, elevated creatinine (129umol/l) paraprotein band in urine and serum. He was referred to YYY on 17.6.15 as multiple myeloma for further evaluation and management.
On admission clinical examination revealed pallor/ normal vitals. He was bedridden with severe pain on movements. Examination of CVS/Respiratory/ Abdomen and CNS was normal. Investigations revealed:

1. العد الدموي الشامل- الهيموغلوبين 108 غملتر, الصفائح الدموية 370 × 109 لتر، عدد كريات الدم البيض 4.41 × 109 لتر التعداد التفريقي لكريات الدم البيض: المعتدلات: 64، الليمفاويات 23، الوحيدات 10، الحمضيات 3%
2. كيمياء الدم- البروتين الكلي 60.5 غملتر، الكالسيوم 2.6 مل موللتر، الكرياتينين 129 ميكروموللتر، باقي النسب طبيعية.
3. الفصل الكهربي لبروتينات الدم (شريط M) 0.9 غملتر.
4. صورة الغلوبولين المناعي للدم- الغلوبولين المناعي جي 5.1، الغلوبولين االمناعي أ 0.25، الغلوبولين المناعي ام 0.17 غملتر.
