관절가동술1)과 스트레칭은 관절 운동범위2)를 회복시키고 어깨의 움직임 조절 단계에서 비정상적인 움직임을 정상에 가까운 움직임으로 만들기 위해 필요하고, 이것은 어깨의 기능적 능력과 환자의 생활 질을 향상 시킬 수 있을 것이다. 본 연구의 목적은 어깨뒤쪽 뻣뻣함3) 있는 충돌 증후군 환자를 대상으로 관절가동술과 스트레칭을 적용한 후, 어깨뼈 주위근육의 근활성도, 위팔어깨 관절의 운동범위, 어깨 통증지수 떤 영향을 미치는지 비교하였다. 23명의 대상자가 각각 2주 동안 실험에 참여하였으며, 모든 평가는 실험 전•후 각각 평가하였다. 근활성도는 어깨 벌림4) 90°, 120°, 150°에서 위등세모근, 중간등세모근, 아래등세모근, 앞쪽 어깨세모근, 앞톱니근5)에서 수집하였다. 위팔어깨 관절6) 운동범위는 전자측각기, 통증지수는 시각적 상사 척도7)를 이용해 각각 평가하였다. 각 군 내 치료 전•후 차이는 대응표본 t-검정, 각 군 간 차이는 독립표본 t-검정을 이용하였으며, 통계학적 유의수준은 α=.05로 하였다. 관절가동술과 스트레칭을 각 군에 적용한 결과 위등세모근, 중간등세모근, 아래등세모근, 앞쪽 어깨세모근의 근활성도와 통증지수가 각각 감소하였으며, 안쪽돌림8)과 바깥돌림9)의 관절 운동범위가 증가하였다. 그러나 앞톱니근에서는 두 군 모두 유의한 차이가 없었다. 그리고 두 군을 비교한 결과, 스트레칭 군에서 위등세모근의 어깨 벌림 150°, 앞쪽 어깨세모근의 어깨 벌림 120°, 150°에서 근활성도가 유의하게 감소하였고, 안쪽돌림의 운동범위는 유의하게 증가하였다. 이러한 결과에 근거해 어깨뒤쪽 뻣뻣함이 있는 충돌증후군10) 환자에게 관절가동술과 스트레칭을 적용하되 스트레칭을 더 많이 사용할 것을 제안한다.
Translation - English
Joint mobilization1) and stretching restore the range of motion (R.O.M)2) and are needed to make abnormal movements near-normal, and thus, they can help improve shoulder functions and patients’ quality of life. The purpose of the present is to have impingement syndrome patients with posterior shoulder tightness3) perform joint mobilization and stretching, and then compare their effects on the patients’ muscle activities near the scapula, range of motion in the glenohumeral joint and shoulder pain.
There were 23 study subjects participating in the experiment for two weeks, and all the assessments were made for both before and after the experiment. The muscle activities were collected from the upper trapezius, middle trapezius, lower trapezius, anterior deltoid and serratus anterior5) at the shoulder abduction angles4) of 90°, 120° and 150°. The range of motion in the glenohumeral joint6) was assessed using an electrogoniometer while shoulder pain was assessed on a visual analogue scale (V.A.S). 7) The differences between before and after the treatment within each group were assessed using a paired t-test while the differences between the groups were assessed using an independent t-test. The statistical significance was set at α=.05. After performing joint mobilization and stretching, each group showed decreases in both the muscle activities in the upper trapezius, middle trapezius, lower trapezius and anterior deltoid, and pain while the range of motion of internal rotation8) and external rotation9) increased. However, there were no significant differences between the groups in the serratus anterior. Moreover, when comparing the results of the two groups, the stretching group showed a significant decrease in the muscle activities with the abduction angle of the upper trapezius at 150° and the abduction angles of the lower trapezius at 120° and 150° while their range of motion of internal rotation increased significantly. Given such results, more stretching exercises are recommended to the impingement syndrome10) patients with posterior shoulder tightness when they perform both joint mobilization and stretching exercises.
