You have been asked by your doctor if you would consider taking part in a research study called Snapshot of Acute Respiratory Distress Syndrome (ARDS) in India by allowing your treating physician to collect data from your medical records. The purpose of this leaflet is to provide you with information about the research study.
Background and Objective of the study
It is an observational study done for research purpose . The main objective of this study is to obtain a better understanding of ARDS in India. It aims to know the spectrum of ARDS, Characteristics and outcome of patients with different types of etiology of ARDS, and to know the prevalence rate of ARDS.
Collection of Data
The data for this research study will be collected from your hospital medical records and sent to a coordinating institution, SHARE (Science, Health, and Research Exchange). You will not need to undergo any additional tests or treatments for purposes of this study. Relevant information from your hospital records will be merely collected.You may not receive direct benefit from this study, but future patients like you diagnosed with ARDS will benefit from the knowledge gained in this study.
The data entry process is password protected. Your privacy will be maintained throughout this study. You will not be identified by your name or any other identifiable information. You will be identified only by a unique number study number that will be linked to your information. Information which could be traced to you will remain confidential and nothing will be disclosed without your consent. Your name or personal data will not be disclosed in any report. Whilst maintaining your privacy, relevant parts of your records may be checked by authorized staff from study personnel, and/or the local Ethics Committee.

मध्यप्रदेश के सतना और उत्तरप्रदेश के चित्रकूट जि़ला जो अंतर्राष्ट्रीय सरहद पर स्थित आदिवासी बहुल्य दस्यु प्रभावित तराई अंचल पर स्थित हैं आज भी राष्ट्र की मुख्यधारा से बहुत दूर हैं और समाज के सक्रिय आखिरी छोर पर खड़े हैं। सतना जि़ले के मनगंवा जनपद पंचायत क्षेत्र की 60 से भी ज़्यादा इन ग्राम पंचायतों का ज़मीनी सच तो यह है कि बियाबान जंगलों में गुम इस अंचल में कानून का राज नहीं है। आम तौर पर पहुंच विहीन इस इलाके में या तो परंपरागत दबंग व्यक्तियों का राज या फि़र डकैतों द्वारा संचालित हकूमत चल रही थी। पटनी जि़ले में यह वही इलाका है जहां के कटनी गांव में एक दशक पहले भारत के तब के राष्ट्रपति महामहिम एपी जे अब्दुल कलाम और तत्कालीन प्रधान मंत्री अटल बिहारी बाजपेयी भी पहुच चुके हैं मगर बावजूद इसके इस बेहद पिछड़े जंगल की नियत नहीं बदली है। कभी 5 लाख के अंतर्राष्ट्रीय दस्यु सरदार शिवकुमार कुर्मी उर्फ ददुआ की तूती भी इसी इलाके में बोला करती थी। आतंक के प्रयाय रहे ददुआ के अंत के बाद भी इलाके में आधा दर्जन से भी ज़्यादा सूचीबद्ध दस्यु गिरोहों का बोलबाला है। सामंती किस्म के स्थानीय सरहंग इन डकैतों का अपने निजि हितों केलिए पोषण करते हैं और फि़र दोनों मिलकर अनपढ़ और निर्धन आदिवासियों का शोषण करते हैं। तराई में डकैत राज का अंदाज़ा इस तथ्य से लगाया जा सकता है कि मध्य प्रदेश पुलिस मुख्यालय ने इस अंचल के नवागांव, धुारकुंडी, सभापुर, भरीगा, सिंहपुर और मंझगंवा समेत 6 थाना क्षेत्रों को दस्यु प्रभावित क्षेत्र (ऐडी ऐरिया) घोषित कर रखा है। इन इलाकों की एक विड़म्बना यह भी है कि केंद्र और राज्य शासन की विविध विकास योजनाऐं बेमानी हैं। दस्युओं और दबंगों की सांठगांठ से उपजे खौफ़ का नतीजा यह है कि महात्मागांधी राष्ट्रीय रोजगार योजना(मनरेगा) जैसी अति महत्वाकांक्षी योजना भी महज़ कागज़ी कवायद बन कर रह गयी है। मनमानी कमीशन और अवैध उगाही के साथ अपहरण के भय से सरकारी अफ़सर तो अफ़सर मैदानी अमले के कर्मचारी भी तराई अंचल को कालापानी से कम नहीं मानते हैं।

1. Subject description
In Satna of Madhya Pradesh and Chtrakoot district of Uttar Pradesh, which is situated on the border of tribes dominated dacoits affected valley circle, even today is far away from the main stream of the country, and are standing at the last active end of the society. The ground truth of more than 60 panchayat area is that, the lost in the deserted forests, this circle has no rule of the law. In the valley the estimate of dacoit rule can be accessed from this fact only that Madhya Pradesh police headquarters have declared the six thanas of this areas of the circle including Naya Gaon, Dhar Kundi, sbha Pur, Barondha, Singh pur and manjh gawan as the dacoit affected area(AD area). One irony of this area is that the central and state government's various ambitious development projects are useless.
With the complicity knot of dacoits and the domineering the results of generated fear is that the ambitious projects like Mahatma Gandhi National Employment scheme(monrega) has only remained as drill. With the fear of the desired commission and illegal collection the official officers and the field staff employees also regard the valley circle not less then Kala Paani.
