This term is used in a document regarding an addendum to a pension policy:

"XX XX ger för övriga anställda möjlighet att teckna ”alternativ ITP” med s.k. frilagd premie."

Haven't managed to find a translation of "frilagd premie". Grateful for any help!

I'd use "opt-out premium" on the basis of the examples at the second link below and this, from the first result at that link:

The ITP plan for the white-collar workers is a non-poolable plan, but if earning more than 10 base amounts (SEK 511,000 in 2010) there is a possibility to opt out of the plan with the employers' consent. The risk parts in these "opt out solutions" could be pooled. Lately we have seen a trend where the employers create their own premium plan for the "opt out solutions".
