Maternal grandmother displayed aberrant behavior at times
Explanation:
It looks like one of two things may have happened here:
1.
The clinician who drafted the report wrote "contacto" when they meant "conducta."
OR
2.
The person transcribing the report from a recording or handwritten document wrote "contacto" instead of the "conducta" that had been originally uttered or handwritten.

In any way, the intended sense seems to be that "grandma was a bit off."

Juan has suggested (below) that "contacto peculiar" may not be a mistake. This is certainly possible. If such is the case, one possible translation of "Abuela materna de contacto peculiar" would be:

relationship with grandmother was anomalous

neutral  Juan Jacob: "Contacto peculiar" me parece correcto... Venga, prefiero "strange ways"...

-> En ese caso, la idea que se quiere transmitir podría ser que el trato de la abuela al nieto fue de alguna manera anómala. Según esta hipótesis, la traducción podría ser: “maternal grandmother [reportedly] behaved in strange ways toward him [at times]."

agree  Toni Castano: "Contacto peculiar" may be unusual as a term, but it´s correct and comprehensible for everyone (the typo, yes, there´s one, is in "según refieren", "según refiere", singular, is right. My agree is for "anomalous", not "aberrant".
