or the Republic of the Two Nations
if you need to emphasize "two nations" although the commonwealth version is more common, in my opinion

Asker: I used the same source to find the answer. This translation seems to be very appropriate.

Otherwise, Lithuanian historians talk about "the Lithuanian-Polish Commeanwealth".

Commonwealth of the Two Nations

Explanation:
The Institute of History has requested me to use this translation. There was an article published on what the official name should be, but can't say where it is located.

I could have sworn I saw Republic in Mr. Kaminskas, but I apparently in my rush saw the asker's comment.
It should be noted that the expression is always 'of THE United States', not 'of United States'. In turn, it should be 'of THE Two Nations'. One always refers to one's own state as The state, The people, etc.
Google supports this with 123,000 hits for 'Commonwealth of THE Two Nations', but only 13,800 for 'Commonwealth of Two Nations'
