acute heart failure on a substrate of liver and kidney failure

Explanation:
καρδιακή κάμψη is in the glossary, but is an imprecise term in Greek that doesn't have a precise medical meaning. I think the rest is self-explanatory.

heart failure on the grounds of/that the ..... suffered hepatic and renal insufficiency

Explanation:
heart failure on the grounds that the patient/deceased suffered hepatic and renal insufficiency (=failure)

Αν και είναι κατα βάση νομικη η χρήση του 'on the grounds'επιχειρηματολογια)δύνεσαι να πείς εναλλακτικώς on the grounds of renal insufficiency

agree  Peter Close: Whenever I have come across επί εδάφους or σε έδαφος in a medical text, I have found 'on the grounds of' or a suitable synonym to be the most appropriate answer.
