German term or phrase: Variable II-Spaltung, IIp nicht akzentuiert
This is in a German medical report for a child who has stenosis of aortic isthmus, they are testing for other heart defect(s) but did not find any.

In the "Klinischer Befund" section:

"...systolisches Intervallregeräusch 2/6 mit p.m. in 3L1, keine Fortleitung. Variable II-Spaltung, IIp nicht akzentuiert, kein Schwirren..."

In my rough draft, I have "Variable splitting of 2nd heart sound" but am not completely sure, but then I have no idea what "IIp nicht akzentuiert" means.

Appreciate suggestions on English translation of above phrase.

Thanks,

Stephen

English translation:splitting of the second heart sound with (normal) respiratory variation
Explanation:
Sorry, I don't know what IIp nicht akzentuiert means.

ABNORMAL SPLITTING OF THE SECOND HEART:
(1) persistently single;
(2) persistent (audible expiratory) splitting, with normal respiratory variation;
(3) persistent splitting without respiratory variation (fixed splitting); and
(4) reversed (paradoxical) splitting.

Wide inspiratory splitting with respiratory variation occurs with right bundle branch block, pulmonary stenosis, or idiopathic dilation of the main pulmonary artery due to delayed activation or prolonged contraction of the right ventricle.

neutral  SJLD: variable splitting of the second heart sound - yes, variable splitting vs fixed splitting of S2 - variable splitting of S2 is not necessarily abnormal in children http://tinyurl.com/87tvmlq
